Cistiti, infezioni e noie alle basse vie urinarie con il dott. Imbimbo!

hello MiciGatti and MiceGatte! in this
video we will talk about a very topic important for the health of our people
cats: these are problems with the urinary tract
like cystitis, stones, infections that often they are difficult to understand for the
owner but they can become a lot dangerous we talk about it together with Dr. Imbimbo
Andrea, veterinarian at the Cuneo veterinary clinic of Cuneo which
he explains how to notice any urinary problems for our kitty e
how to act in the best way. The Doctor Imbimbo will make a brief
introduction on how the apparatus works urinary tract of the cat and subsequently the
I will report to the most common questions that do the cat owners they have
urinary tract problems. I leave therefore the word to Dr. Imbimbo. Hello everyone
my name is Andrea Imbimbo, I thank Elisa from cat cat for the pleasant invitation to
this meeting, many of you will know me
probabimente as in ImbimboVet for my instagram profile, so sui
social where I still deal with topics related to the profession.
Today we will talk about the problems of lower urinary tract in cats, problematic
sometimes they can be an urgency and therefore it is important to recognize immediately
so to bring your own quickly animal to the vet to understand
however these issues. You have to make an incipit for how long
concerns let’s say the normal anatomy of the cat, as you can see in the photo the anatomy of the cat as well as the human being and the dog is made up of two kidneys
which are say the bodies appointed to clean the blood and metabolize the
slag that is present inside but also to produce hormones,
but let’s say that summing up they produce urine,
the urine which is then transported two small tubes of ureters up to
bladder level, the bladder that has function say to contain urine and
then the last part of the urinary tract and urethra which is a lot
important because we say it is part lower urinary tract together with the bladder,
urethra that in the initial part has a small sphincter which is controlled by the
let’s say voluntary innervation that therefore it allows the animal to choose
when or not to urinate. One of the important differences that I would like to point out is that the male urethra is clearly longer than that of the female and therefore this
predisposes to various problems. Thank you Dr. Imbimbo for this beautiful
explanation, but let’s start those now questions and the first question is: how do you get there
realizes that the cat has problems at urinary tract?
then there are several signs that can make you suspect
problems with the lower urinary tract of the your cat friend then first of all there
it will definitely be pollakiuria, therefore a frequent emission of urine,
another is the straguria, therefore the emission of urine accompanied by
important pain, in in this case the cat will have attitudes
details therefore will manifest pain in urination, with vocalizations,
or try to relieve the pain licking the perineal area or some
times the area in correspondence let’s say therefore the most caudal part of the bladder
abdomen, however it will be a cat that usually
he will not be fondled willingly. More problems that are not from
definitely underestimate urination inappropriate, so in
different areas of the house and no longer in the litter, this may or may not be
a problem of the lower urinary tract. Another instead important signal of problem
in the lower urinary tract it is definitely hematuria therefore mention of urine with
blood traces then with blood, in in this case the urine will have a color
typically reddish. But therefore when am I aware of these things should I immediately run to the vet? and do I have to worry? Surely everyone
these symptoms that I listed in previous answer are not from
underestimate therefore need however a visit from
your trusted vet. One of the more severe symptoms and therefore what
perhaps we say it requires a quick visit from the vet is anuria, so when
your pet shows up more times in the litter, but can not put
urine. This is a problem that can also lead to deadly consequences
even if you wait too long, because the animal meets
intoxication, so it fails to emit urine, some are reabsorbed
harmful waste and therefore goes intoxicating. In this case there are there are gods
trick to understand who the cat is anuric: first of all it goes several times
as I said in the litter to try to urinating without emitting urine, another
who has a lot of pain becomes a little cat therefore negotiable even if before it was
sweet often becomes even aggressive and another way is there
a little manual skill that now you I will show in the next video where you can
notice a bladder let’s say much fuller than usual.
Here I am with Angus and I wanted just explain this simple
manuality, to go looking for palpate the bladder and see if it is large
size or less. You have to take it first the cat and place your hands
in the most caudal part of the abdomen then try to touch your fingers by touching
through the abdomen, so if in this so here you hear or feel one
structure similar to a full balloon of water most likely that is the bladder and
so we will have to run, a reason in more to run to the vet because
it will mean that your cat is obstructed.
Obviously they will have to be present also the other typical symptoms therefore the
anuria, the cat who tries to urinate more times without success. Say hello to a
little Angus! But male cats and cats females can have the same
urinary tract problems or the thing a little change? For diseases of the apparatus
lower urinary then bladder and urethra they can also affect both the
male than female, there isn’t one predisposition of sex.
Let’s say the factors that can predispose to problems of this
level is certainly sterilization, unfortunately there are more spayed cats
predisposed to these problems, another factor
predisposing is obesity, the band in mainly affected by cats
for these pathologies between 1 and 10 years. The let’s say tumor and inflammatory pathologies
occur in both male and female female, the male is more subject to one
typical problematic is obstruction urethral, this is because the
length of the urethra is greater than the female and a
another reason is that the male urethra does goes to tighten at the level of the penis, where
therefore it becomes more subject to possible obstructions from both mucous plugs and from
small calculations that occur in the its course. The vet you examine
will do to my cat and how it will choose which ones exams do? Since clinical symptoms
I’m not always attributable to a specific cause,
a clinical visit will be required accurate, but often it will also be
equally necessary to take exams collateral, therefore investigations of
laboratory or other. now I will list you let’s say the main ones
exams you can choose from of course not all of them are
indispensable, however, some will be discretion of the colleague. Surely
an exam that will be the cornerstone in the lower urinary tract problems is
definitely the urine test, exam of the urine it involves
analyze your pet’s pee e with different methods we say that the
most important thing is the type of sampling is certainly the one of choice is cystocentesis, so go with the ultrasound see to evaluate the
bladder and go to collect directly with a needle urine, so
sterile, don’t worry if yours veterinarian will offer you this method
because it is absolutely non-traumatic, it doesn’t bother your cats, it doesn’t
bad for the animal, but it is essential for to be able to perform a complete urine test
so also go to evaluate if there is a bacterial infection in progress because not
there will be a contamination of the whole rest, in the remaining part we say
of the urinary tract, the others withdrawal methods then the
catheterization by urination or by peeing in the litter box
let’s say they are less effective in this case to evaluate a bacterial infection
of the bladder. With the urine test we have several important parameters including ph,
the possible presence of crystals, let’s go see the sediment if there is a
active infection, an inflammation therefore with presence of anthrophilic bacteria
and other indications very very much important
to do proper therapy and one therapy aimed at the problem.
Another exam that your vet can offer you is the blood test,
we say it does not always result necessary, it may be necessary when
a concomitant is suspected kidney problems or an obstruction,
in this case we will evaluate let’s say a few parameters we need
hence potassium, abumia and creatinine which allows us to go and evaluate the
kidney function and also the degree of intoxication following a urethral obstruction. Next step to investigate the
problem is diagnostic imaging, then radiography or ultrasound or in
some cases endoscopy. I’ll talk about one after the other then radiography
I honestly don’t feel like recommend it a lot because it gives few
information: if we suspect a bladder problem or urethral can give indications, in the
if presence is suspected of a calculation that yes, because if
were radiopaque you would see radio well graphically. Let’s say though maybe
ultrasound is the diagnostic investigation par excellence, let’s say the first choice,
at least for myself, because first of all it is very fast, we had already done it before to do cystocentesis, so already that
there we can take a look at to the bladder and kidneys, to the urethra in the
visible part without going to stress the animal making an x-ray e
it also gives us useful information for both the possible presence of calculations and then a
difference of radiography makes us see the bladder content we say so
better then gives us directions more. Cystoscopy I feel like
advise against it enough, unless I suspected a particular issue
as a possible neoplasm that in that case we have to go sample
the problem, otherwise it does not have all of these advantages and it is very inquisitive,
and needs an anesthesia. every case I made you a list let’s say summary
of the main diagnostic methods and of course the choice
the colleague will have to do it according to suspect and to address so
then corrected a therapy. But to this point then let’s talk about prevention:
how do I prevent problems at urinary tract of my cat? and once
solved the immediate problem what I have to do?
what should i watch out for? What that I must stress very important is that it is
bacterial infection is very rare, too if you think not, instead it is very rare in the
cats over 10 years is a little more likely, but in cats under ten
years we say that only 5 percent of they are affected by the infection
bacterial active in the lower urinary tract, we say that the antibiotic already results
therefore it is unnecessary to go to the pharmacist a ask for an antibiotic
we say that most likely not will solve the problem.
in 60 percent of cases the cause is idiopathic, so it is not known
what the problem is and you just have to let’s go for therapy
symptomatological, let’s say that the cat is an animal that sees many
therefore predisposing causes many causes which facilitate the formation of infection e
an inflammation of the lower streets urinary and here the role of
owner is fundamental for the prevention,
first of all a very cause important to predisposition to these
problematic is obesity, therefore a cat obese is predisposed for example to a
obstruction or has an inflammation of these low ways, so keep the
cat in a correct fit weight is fundamental, among other things, the advice that
I want to give to the owner is to give always a wet diet to the animal or
however, stretch it with a little water to guarantee a water supply in any case
enough during the day, cats they are animals that usually do not drink
a lot then try to stimulate the cat to drink is useful.
another thing i want to emphasize is about do not give specific diets to yours
animal without consulting first vet or otherwise a prescription
veterinary: diets for example acidifiers that in some cases serve
dates without a real need can bring problems.
Let’s say the main cause basically infections
lower urinary tract maybe it’s stress, hence the reduction of stress in the
cat is essential, stress that can be brought by new ones
animals, new people in the house, for example a child or even a
overcrowding or a sudden change of the diet. Another thing
very important is to grant one cat litter in the house, so if
you have two cats at least two litters, if not even three, that’s why
prevents competition for litter itself and also prevents urination
inappropriate, so pee around home. As a therapy I will not be able to dwell
too much, point 1 because every possible problematic needs therapy
different and therefore requires a visit e the then therapeutic proposal of a
colleague, point 2 because in most part of the cases, 60 percent, like
I said before there is no real cause, the cause is idiopathic, it is not known, and therefore it goes
treated the symptom. One piece of advice I can give is that there
they are products commercially available nutritional additives
which contain internal GAG that we say they are substances that have an effect
protective, anti inflammatory towards the bladder mucosa and therefore can
let’s make it more rare or otherwise disadvantage the formation of a
problems with the lower urinary tract. Now we have arrived at the time of
