[UPDATED] OPT: Mailing your Application to USCIS 4/9


When you receive your OPT I-20 from OIE, it’s time to mail your application to USCIS. Remember that USCIS must receive your application before the end of your 60 day grace period and within 30 days from the date your OPT I-20 was issued. Here’s what you need to include in your application to USCIS: 1) 2 passport style photos of yourself 2 inches by 2 inches with your name and I-94 number written lightly on the back using pencil 2. A check for $410 payable to U.S. Department of Homeland Security 3. Original I-765 form. Paperclip the photos and the check to your I-765 4. A Copy of your OPT I-20 (your I-20 must be less than 30 days old) 5. Copy of your passport ID page showing your picture and the expiration date 6. Copy of your I-94. The I-94 confirms your presence in the USA. You cannot apply for OPT from outside of the country. 7. CPT Employment history page from SEVIS (OIE will provide this if applicable) 8. If you have had other work authorization cards previously, be sure to include copies of them 9. If you had a change of status, include a copy of your approval notice 10. Finally, you can choose to receive electronic notifications about your OPT application. To choose this option, complete form G-1145 and attach it to your I-765 It’s a good idea to make a copy of the completed application for your personal records. Now it’s time to double check everything and mail your application to USCIS! A word of advice: Make sure everything is right before you mail your application. Trying to make any changes to your application after mailing it off could end up increasing the processing time. If you’re using the US Postal Service (regular
mail), send your application to USCIS at P.O. Box 660867, Dallas, TX 75266. If you choose to mail your application by
Express Mail or a private carrier, address your application to USCIS Attn: AOS, 2501
S. State Hwy 121 Business, Ste 400, Lewisville, TX 75067. When USCIS receives your application, they will mail you a receipt notice. This usually happens 2-3 weeks after you mail your application. The receipt notice will have the tracking number for your application. You can track your application online at the USCIS portal. Usually, it takes 60-90 days for USCIS to approve
